TOURNAMENT REGISTRATION FORM

Before starting your registration, you will need:

• Proof of membership for each angler and crew
• Boat Insurance Form and Policy Number
• All Anglers details (phone, email, DOB, shirt size, QGFA or QFAA club)
• Skipper and Deckie details (phone, email, QGFA or QFAA club)
• Associate Membership available for Male Skippers and Deckies
